Case Management is a community-based service for individuals with Medicaid living with a severe and persistent behavioral health diagnoses. Case managers are part of the collaborative coordination of the individual’s treatment plan, focusing on improving self-sufficiency through planning, connection to community resources, advocacy, coordination, and monitoring of activities. <\/p>

The purpose of case management is to create opportunities and increased independence for the individual. <\/span><\/p>\n
